Job summary

Provides after-hours hospice nursing care including patient assessment, coordination with teams, and home visits.

Responsibilities

Assess and identify patient needs after hours in accordance with the hospice concept of care. Coordinate with the interdisciplinary team to provide patient and family care while maintaining continuity of care.
