Job summary

Provides after-hours hospice nursing care, assesses patient needs, coordinates with the interdisciplinary team, performs triage, and conducts home visits as required.

Responsibilities

Assess and identify patient needs after hours in accordance with the hospice concept of care. Coordinate with the interdisciplinary team to provide patient and family care, including performing triage or home visits as assigned.

Requires graduation from an accredited school of nursing and current Michigan licensure as a Registered Nurse. Must have a minimum of one year of hospice RN experience and a valid driver's license.
