Job summary

The Nurse Practitioner, Clinical Lead will conduct virtual medical evaluations, develop and manage evidence-based care plans for patients with cardiometabolic conditions, act as the primary prescriber in Full Practice Authority states, manage device-triggered alerts, and contribute to building scalable clinical workflows in a remote, digital health setting.

Responsibilities

The NP will conduct virtual medical evaluations, create and manage evidence-based care plans, prescribe medications, and collaborate with the Medical Director to build scalable clinical workflows, all within a remote telehealth environment.

Cardiology cardiometabolic care care coordination Chronic Disease Management clinical judgment clinical protocols Clinical Protocols Development digital health tools EHR documentation Endocrinology Internal Medicine Medication Titration population health management prescribing medication remote patient management Remote Patient Monitoring telehealth

Candidates must hold an active NP license, board certification, and have at least 2 years of experience in primary care or related fields, with comfort in telehealth and digital health tools. Bilingual proficiency in Spanish is a plus, and US residency is required.
