Job summary

The role involves providing home-based nursing care to patients as an RN, working autonomously within legal scope, coordinating with a multidisciplinary team, educating patients and families, monitoring patient conditions, administering medication, and documenting care, with a focus on quality, safety, and patient outcomes in a flexible schedule.

Responsibilities

The Registered Nurse will provide one-on-one home-based nursing care, including performing OASIS assessments and developing individualized care plans. They will also collaborate with an interdisciplinary team and physicians to ensure continuity of care and educate patients and families on disease processes.
