Job summary

A registered nurse provides home-based nursing care, develops care plans, administers medication, and educates patients and families in a home care setting, working autonomously and collaboratively with interdisciplinary teams.

Responsibilities

Provide one-on-one skilled nursing care and OASIS assessments to develop individualized care plans for patients in their homes. Coordinate with interdisciplinary teams and physicians to manage patient recovery and educate families on disease processes.

Qualifications

Must be a Registered Nurse with a current state license and at least one year of experience in an acute care setting. A valid driver's license, auto insurance, and the ability to meet physical demands are required.
