Job summary

Seeking a Hospice RN Case Manager to provide skilled nursing care to terminally ill patients, coordinate end-of-life services, and develop personalized care plans in-home and facility settings, ensuring comfort, dignity, and quality of life.

Responsibilities

Manage a primary hospice caseload and provide skilled professional nursing care to patients, families, and caregivers as prescribed by physicians. Ensure care complies with Medicare hospice requirements, Ohio hospice licensure laws, and agency policies and procedures.

Qualifications

Applicants must be graduates of an approved nursing program and hold a valid Ohio Registered Nurse license; a bachelor’s degree in nursing is preferred. Two to four years of direct patient care experience is sought, with hospice, case management, and end-of-life diagnosis experience preferred.
