Seeking an Advanced Practice Provider to deliver high-quality, patient-centered care at skilled nursing facilities, with responsibilities including daily rounds, medication prescriptions, acute issue management, and communication with staff, in a flexible schedule that excludes on-call duties.

Responsibilities

Provide high-quality, patient-centered care to residents in skilled nursing facilities through daily rounding and treatment planning. Coordinate with facility nursing staff and ensure compliance with clinical and regulatory standards.

Qualifications

Must be a board-certified Nurse Practitioner or Physician Assistant with an active U.S. license. Qualified specialties include Internal, Family, Emergency, General Practice, Physical Medicine and Rehabilitation, or Geriatric Medicine.
