Job summary

Provide compassionate, hands-on care to hospice patients, ensuring comfort, dignity, and independence while supporting their end-of-life journey under RN supervision.

Responsibilities

Provide compassionate, hands-on personal care to hospice patients in their homes or facilities to ensure comfort and dignity. Collaborate with an interdisciplinary team and report changes in patient conditions to the registered nurse.

Qualifications

Assistance with daily living CNA HHA Homemaking support Mobility support Nurse Aide Nutrition support Observation reporting Patient care

Requires approved hospice aide, home health aide, or nurse aide training and competency. Candidates must have the ability to travel within the assigned service area.
