Job summary

Responsibilities

The Case Manager identifies and coordinates care for terminally ill patients, leading the interdisciplinary team to develop and implement individualized plans of care. They provide direct nursing interventions, educate families on disease processes, and ensure documentation meets all regulatory and accreditation standards.

Qualifications
