Job summary

A field-based registered nurse role assisting members with complex needs through in-home assessments, care coordination, and community resource linking, primarily in Hudson County, NJ.

Responsibilities

Conduct in-home assessments for members with complex needs and develop individualized care plans to support their health and safety. Collaborate with interdisciplinary teams and providers to ensure coordinated, holistic care and document all activities in the electronic health record.

Qualifications
