Job summary

RN Care Manager coordinates holistic, patient-centered care for high-risk and chronic illness patients by assessing, planning, and monitoring services to improve health outcomes and reduce preventable hospital visits, working closely with a multidisciplinary team.

Responsibilities

The RN Care Manager coordinates holistic care for patients with complex health and social needs by assessing, planning, and monitoring individualized care plans. They collaborate with multidisciplinary teams to improve clinical outcomes and prevent avoidable hospital admissions through patient education and resource coordination.

Qualifications

Candidates must be a Registered Nurse with an active, unrestricted Texas license and current BLS/CPR certification. The role requires 3-5 years of acute clinical experience and 1-2 years of case or care management experience.
