Job summary

A Hospice RN Care Manager provides compassionate nursing care to hospice patients at home and in care facilities, assessing needs, developing care plans, and coordinating interdisciplinary team efforts.

Responsibilities

Provide compassionate, patient-centered nursing care to patients in their homes and care facilities. Coordinate with an interdisciplinary team to develop and adjust individualized plans of care while maintaining accurate electronic health records.

Qualifications

Care Planning Communication Documentation Nursing Patient Care Teamwork Time Management

Requires a graduate degree from an accredited nursing program and a current RN license in the State of Ohio. Candidates must possess a valid driver's license, reliable automobile insurance, and the ability to travel throughout the assigned territory.
