Job summary

Provide hands-on, compassionate end-of-life patient care, assisting with personal needs, mobility, nutrition, and reporting health changes under RN supervision.

Responsibilities

Provide compassionate, hands-on personal care and mobility assistance to hospice patients in their homes or facilities. Observe and report changes in patient conditions to the registered nurse while working as part of an interdisciplinary team.

Qualifications

Activities of daily living assistance Compassionate care Homemaking support Meal preparation Mobility assistance Nutrition support Observation and reporting Patient care

Requires approved hospice aide, home health aide, or nurse aide training and competency. One year of direct patient care experience is preferred, and the ability to travel within the assigned service area is mandatory.
