Job summary

Seeking a Licensed Practical Nurse (LPN) to provide skilled nursing visits in-home for adult clients, administering medications, wound care, and treatments as part of an interdisciplinary team.

Responsibilities

Provide one-on-one skilled nursing visits to adult clients in their homes, including administering medications and wound care. Coordinate care with the interdisciplinary team and maintain accurate documentation in the electronic medical record system.

Qualifications

Requires a current New Jersey Licensed Practical Nurse (LPN) license and a minimum of one year of recent clinical experience. Candidates must have reliable transportation to travel throughout the assigned territory.
