Responsibilities

The Hospice Registered Nurse Case Manager will build compassionate relationships with patients and families to provide comfort and dignity. They will also empower caregivers through education and collaborate with the care team to ensure high-quality end-of-life support.

Qualifications

Candidates must hold a current RN license in the state of practice and possess at least two years of registered nursing experience. Additionally, a current driver's license and CPR/BCLS certification are required.
