Job summary
This position works collaboratively with other members of the interdisciplinary team to assure delivery of high quality nursing service in accordance with the physician’s orders and with the agency’s programs and policies. Services are provided in the home and in the patient’s place of residence. Responsibilities include assessment, care planning, patient education, coordination with physicians and community resources, documentation, and adherence to policies. The nurse must demonstrate skills in clinical care, communication, documentation, teaching, and safety.
Responsibilities
The Registered Nurse will perform comprehensive patient assessments and develop individualized care plans in a home health setting. They will coordinate with an interdisciplinary team to ensure high-quality nursing care and compliance with physician orders and agency policies.
Qualifications
Candidates must be a graduate of an approved school of professional nursing and hold a current Registered Nurse license. Two years of professional nursing experience or community health experience is preferred.
