This remote role involves significant local travel for in-home visits and requires an active New Jersey RN license. Responsibilities include evaluating member needs, developing care plans, coordinating services, monitoring progress, and educating members and families.

Responsibilities

Qualifications

Candidates must hold an active New Jersey Registered Nurse (RN) license and possess a degree from an accredited school of nursing or a Bachelor's degree in Nursing. The role requires 2 to 4 years of related experience and the ability to travel locally for in-home member visits.
