Job summary

A Registered Nurse (RN) provides home-based nursing care, manages patient conditions, educates families, and collaborates with an interdisciplinary team to ensure quality patient outcomes in a home care setting.

Responsibilities

The Registered Nurse will provide skilled, one-on-one nursing care in patients' homes as directed by attending physicians. Responsibilities include performing OASIS assessments, developing individualized care plans, and educating patients and families on disease processes.
