Job summary

Adult’s Social Worker position supporting adults with long-term conditions and complex needs within a community setting. Responsible for assessments, safeguarding, care planning, and multidisciplinary collaboration to promote independence and wellbeing.

Responsibilities

The social worker will manage a community-based caseload of adults with long-term conditions, conducting statutory assessments and safeguarding interventions. They will collaborate with families, health professionals, and partner agencies to develop person-centred support plans that promote independence and wellbeing.

Qualifications

Candidates must hold a recognised social work qualification and be registered with Social Work England. Significant post-qualified experience in adult social care, including knowledge of the Care Act 2014 and Court of Protection processes, is required.
