Job summary

The Care Management Coordinator RN position involves overseeing processes in care management, performing utilization review, coordinating discharge and care plans, and ensuring compliance with payment and CMS requirements. Requires RN licensure, 3-4 years clinical nursing experience, and ability to work with multidisciplinary teams in a patient-centered environment.

Responsibilities

Oversees evidence-based practice at the bedside and manages government and commercial utilization review processes. Coordinates comprehensive discharge plans and collaborates with multidisciplinary teams to ensure smooth patient transitions between levels of care.

Qualifications

Communication Computer skills Negotiation Nursing Problem Solving Utilization Review

Requires an Associate's Degree or Diploma in Nursing, with a BSN required within 5 years of hire, and BLS/CPR certification. Candidates should have 3-4 years of clinical nursing experience and strong communication and negotiation skills.
