Job summary

Provide compassionate nursing care to homecare patients, monitor patient progress, update care plans, and coordinate with healthcare team to support positive health outcomes.

Responsibilities

The Licensed Practical Nurse will follow individualized care plans and monitor patient progress while reporting changes to the care team. They will also coordinate with physicians and families to ensure high-quality care and prevent avoidable hospitalizations.

Qualifications

Candidates must hold a current state license as a Licensed Vocational or Practical Nurse and have at least one year of clinical experience. A valid driver's license and the ability to lift up to 50 pounds are also required.
