Job summary

Provide psychosocial assessment, counseling, and intervention services to patients, collaborating with healthcare teams to promote mental health, manage crisis situations, develop care plans, and link community resources.

Responsibilities

Assess patients’ psychosocial needs and develop, implement, document, and adjust care plans that support continuity of care and mental health. Provide counseling, crisis support, advocacy, referrals, and education while collaborating with patients, families, healthcare teams, and community agencies.

Qualifications
