Job summary

Supporting safe hospital discharges and promoting recovery and independence for adults in Lancashire as an Occupational Therapist, working with hospital teams, families, and community partners to deliver person-centered care.

Responsibilities

The Occupational Therapist will support safe, timely, and person-centred hospital discharges by working closely with health colleagues and families. They will promote recovery and independence while embedding strengths-based practices within the Adult Social Care service.

Qualifications

Discharge Planning Hospital Discharge Independence Support Occupational Therapy Patient Advocacy Person-centered Care Recovery Promotion Team Collaboration

Applicants must be registered with Social Work England (SWE) or have applied for registration. The role requires the provision of a car for business use and the ability to pass a Disclosure and Barring Service (DBS) check.
