Job summary

Community Care seeks a full-time Case Manager to support elderly and adults with disabilities in community-based care. The role involves managing caseloads, conducting assessments, creating care plans, coordinating services, and maintaining documentation, primarily based in Wisconsin counties with local travel requirements.

Responsibilities

Manage an assigned caseload in collaboration with an RN Case Manager, complete member assessments, care plans, and functional screens. Coordinate care, monitor service quality, and maintain documentation according to established standards.

Qualifications

A bachelor’s degree in human services or a closely related field is required; a non-human-services degree may be substituted with at least three years of relevant experience. One year of experience with older adults or adults with disabilities in a social service capacity is preferred, along with eligibility for long-term care functional screener certification, a valid driver’s license, personal vehicle, and required insurance.
