Job summary

The Care Manager / Patient Engagement Specialist builds relationships with patients through outbound calls, using motivational interviewing and clinical best practices. Responsibilities

The Care Manager will conduct high-volume patient outreach to build rapport and drive health outcomes through consistent engagement. They are responsible for managing patient panels, developing SMART goals, and maintaining accurate clinical documentation to support care plans.
