Job summary

Registered Nurse (RN) in senior living and home health care providing skilled nursing, assessments, medication management, wound care, and care coordination for adult and geriatric clients in assisted living, independent living, and intermediate care facilities.

Responsibilities

Provide skilled nursing visits to clients in senior living communities, including comprehensive assessments and individualized care plans. Coordinate care with an interdisciplinary team and maintain accurate clinical documentation using an electronic medical record system.

Qualifications

Requires a current New Jersey Registered Nurse (RN) license and at least one year of recent, verifiable clinical nursing experience. Candidates must have reliable transportation to travel throughout Somerset, Middlesex, Monmouth, and Union Counties.
