Job summary

Provides patient care support including hygiene, nutrition, ambulation, elimination, and safety services as directed by Charge Nurse. Documents patient info, takes vital signs, and responds to resident needs, aiming for quality care in a long-term care setting.

Responsibilities

Provide direct resident care, including assistance with hygiene, nutrition, mobility, elimination, safety, admission, and routine monitoring, following nurse instructions and established guidelines. Document care accurately, respond to resident needs, maintain supplies, communicate relevant information to the Charge Nurse, and coordinate with the care team.

Qualifications

Assisting with activities of daily living Communication skills Documentation Infection control Patient care Repositioning Use of medical equipment Vital signs

An active state Certified Nursing Assistant certification is required; a high school diploma and long-term care experience are preferred, and CPR certification is required or must be obtained within six months. Candidates should have strong communication and organizational skills and be able to stand, walk, lift, transfer, and move residents and equipment using proper techniques.
