Job summary

Provide nursing care and case management to home health clients as an LPN, following care plans and regulatory standards, reporting to the Clinical Leader, and participating in team and on-call duties.

Responsibilities

The Licensed Practical Nurse provides direct nursing care and case management to clients in their homes according to the established plan of care. They also participate in interdisciplinary team meetings and provide education to patients and their families.

Qualifications

Candidates must hold a current LPN license and possess a valid driver's license with reliable transportation for daily travel. At least 12 months of facility or agency nursing experience is preferred.
