Job summary

Seeking experienced Registered Nurses for future Hospice RN Case Manager roles, providing compassionate end-of-life care, coordinating comprehensive patient support, and collaborating with interdisciplinary teams in the Metro Detroit area.

Responsibilities

Provide skilled nursing care and compassionate support to hospice patients and their families, promoting comfort, dignity, and quality of life. Coordinate comprehensive care for an assigned caseload and collaborate with patients, families, physicians, facilities, and interdisciplinary team members.

Applicants must graduate from an accredited nursing program, hold a current Registered Nurse license, and have at least one year of nursing experience in a clinical care setting.
