Registered Nurse (RN) for hospice care providing assessment, planning, and support to terminally ill patients and their families, ensuring compassionate, personalized care in compliance with healthcare standards.

Responsibilities

The RN Care Manager will assess patient needs and develop individualized care plans to provide high-quality hospice services. They will collaborate with an interdisciplinary team and families to ensure care is delivered according to professional nursing standards.

Qualifications

Candidates must hold a current Ohio Registered Nurse license and have graduated from an accredited nursing program with at least an Associate of Science in Nursing. A valid driver's license, reliable transportation, and the ability to travel within the service area are required.
