Job summary

The Nurse Practitioner (NP) provides direct patient care including assessments, diagnosis, treatment, and coordination in various healthcare settings. Requires collaboration with healthcare teams, independent practice, and delivering comprehensive geriatric and general patient care.

Responsibilities

The Nurse Practitioner provides direct patient care in medical centers, nursing homes, and home settings, including geriatric assessments, physical exams, and treatment planning. They collaborate with a multidisciplinary team to manage patient care, document progress, and facilitate transitions between care levels.

Qualifications

Candidates must hold an ARNP or similar advanced nursing degree with at least 2 years of clinical experience in Family Medicine, Internal Medicine, or Geriatrics. A current Nurse Practitioner certification in the state of practice and BLS certification are required.
