Job summary

On-call RN Case Manager responsible for providing hospice services, assessing patient needs, and coordinating care during on-call hours in a home or community setting. Requires compassion, clinical expertise, and communication skills.

Responsibilities

Provide on-call hospice nursing care, including assessing, planning, implementing, and evaluating patient and family needs in collaboration with the triage team. Coordinate admissions, medications and supplies, care plans, clinical documentation, and hospice aide supervision, while reporting patient status and on-call activities to clinical leadership and physicians.

Qualifications

Care Coordination Clinical Judgment Communication Hospice Care Nursing Patient Care

Requires an unrestricted RN license in the state of practice, a valid driver’s license with proof of auto insurance, and two years of RN experience. Two years of hospice or palliative nursing experience is preferred.
