Job summary

The Case Manager provides community-based behavioral health services, including assessment, resource coordination, and crisis intervention, to clients. The role requires developing personalized service plans and supporting clients in achieving recovery and stability through resource linkage and crisis management.

Responsibilities

Manage a caseload of up to 30 clients, assess their needs, and develop measurable, strength-based service plans grounded in Recovery Model principles. Coordinate access to housing, healthcare, employment, and other supports; monitor progress, provide crisis intervention when needed, and maintain timely service, billing, and authorization documentation.

Applicants must meet one of the listed education, nursing, or relevant human-services experience pathways, including a bachelor's degree in a related field, registered nurse status, or specified high-school education and experience combinations. Required clearances include Pennsylvania child abuse, criminal, and fingerprint-based federal criminal history checks, as well as verification of no Medicaid or Medicare exclusion.
