Job summary

Provide in-home hospice nursing care for patients with end-of-life needs, collaborating with a multidisciplinary team to support patient comfort and family education.

Responsibilities

Provide comfort-focused hospice nursing care to patients in their homes or care facilities, typically making four to six visits per day. Advocate for patients and families, coordinate with an interdisciplinary care team, and educate patients, families, and caregivers about hospice care.

Qualifications

Applicants must have an associate or bachelor's degree in nursing, a valid RN license, and at least one year of hospital nursing experience. Reliable transportation is also required.
