Job summary

Provides care to hospice patients under the supervision of a registered nurse or physician, including administering medications, monitoring patient reactions, and maintaining documentation. Requires LPN licensure, BLS certification, and at least one year of experience.

Responsibilities

Provides direct patient care under the direction of a registered nurse or physician in accordance with established care plans. Maintains communication with the medical team and family while ensuring accurate documentation and safe medication administration.

Qualifications

Requires graduation from an accredited school of nursing and at least one year of experience as a licensed practical nurse. Must hold a valid LPN license and Basic Life Support (BLS) certification.
