Job summary

Provide skilled nursing care in patients’ homes, monitor progress, educate patients and caregivers, coordinate with healthcare team, and help prevent hospitalizations.

Responsibilities

Provide skilled nursing care in patients' homes according to individualized care plans and physician orders. Monitor patient progress, document interventions, and collaborate with the RN Case Manager to ensure coordinated care.

Qualifications

Requires an active Massachusetts LPN license and at least one year of clinical nursing experience. Candidates must have a valid driver's license, reliable transportation, and the physical ability to lift 50-100 lbs.
