Responsibilities

The Registered Nurse will conduct comprehensive patient assessments and develop individualized care plans while performing skilled nursing visits in patients' homes. They are responsible for coordinating with the interdisciplinary team, managing medications, and maintaining accurate, Medicare-compliant clinical documentation.

Qualifications
