Job summary

This case management role involves coordinating care for members with complex health and social needs in Delaware, including conducting home and facility visits, assessing needs, planning and authorizing services, facilitating care transitions, and educating members or caregivers while working within regulatory and organizational guidelines.

Responsibilities

Serve as the single point of contact to coordinate care needs for members across service delivery systems and community supports. Conduct regular in-home and nursing facility visits to assess, plan, and evaluate care for members with complex health and social needs.
