Job summary

A hospice RN Care Manager provides compassionate end-of-life care, assessments, care planning, patient and family education, and interdisciplinary team coordination in a field-based role requiring local travel throughout Franklin County.

Responsibilities

The RN Care Manager assesses patients to develop and implement individualized plans of care while providing skilled nursing interventions. They also collaborate with the interdisciplinary team and educate families on hospice resources and comfort measures.

Qualifications

Candidates must be graduates of an accredited nursing program and hold a current, unrestricted Registered Nurse license in Ohio. A valid driver's license, CPR certification, and the ability to travel to patient homes are required.
