Job summary

Provide home-based nursing care as an LPN, focusing on patient recovery, education, and safety, working independently with a flexible schedule in a supportive team environment.

Responsibilities

Provide physician-directed skilled nursing care in patients' homes, monitor their conditions, administer prescribed medications, and report changes to the case manager. Educate patients and families, complete clinical documentation according to agency and Medicare guidelines, and follow agency policies and procedures.

Applicants must hold a current Licensed Practical Nurse license in the state of employment and have at least one year of LPN experience in an acute care setting; home care experience is preferred. A valid driver's license, auto insurance in the applicant's name, and the ability to meet the role's physical demands are also required.
