Job summary

Join Residential Home Health and Hospice as a Weekend RN providing compassionate, patient-centered end-of-life care to patients in their homes, collaborating with an interdisciplinary team, responding to urgent needs, and documenting care with clinical judgment and critical thinking.

Responsibilities

Deliver compassionate, patient-centered hospice care by performing urgent assessments and managing symptoms in patient homes. Collaborate with an interdisciplinary team to provide end-of-life support and guidance to patients and families during weekends and crisis situations.

Qualifications

autonomy clinical judgment driving emergency response EMR documentation interdisciplinary collaboration pain management patient care patient education symptom management

Requires an active Registered Nurse license and at least two years of clinical experience. Candidates must possess a valid driver's license and the ability to travel to patient homes.
