The provider will deliver primary care services to residents in long-term care facilities, including performing physical exams and diagnosing health conditions. They are also responsible for developing individualized care plans, prescribing medications, and collaborating with interdisciplinary teams.

Qualifications

Candidates must hold an active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license in Tennessee. A minimum of 6 months of experience in long-term care or a skilled nursing facility is required.
