Job summary

Licensed Practical Nurse in rehabilitation hospital providing patient assessments, interventions, education, and care planning with a focus on recovery and quality of life for inpatients.

Responsibilities

Assess patients, identify care needs, initiate appropriate nursing interventions, and provide age- and condition-appropriate care and education throughout rehabilitation and before discharge. Collaborate with the interdisciplinary team, maintain and revise care plans and medical records, and complete quality improvement assessments and activities.

An active Wisconsin or compact-state LPN license and an associate degree are required; AHA Basic Life Support certification must be obtained within 30 days of hire.
