The RN Home Health Case Manager oversees patient care in a home health setting, conducts assessments, develops care plans, documents interactions, and collaborates with healthcare teams to ensure quality patient outcomes.

Responsibilities

The RN Home Health Case Manager conducts initial patient assessments, establishes and updates care plans, and provides direct nursing care in home settings. They also facilitate discharge planning and maintain ongoing communication with physicians and the healthcare team to ensure continuity of care.

Qualifications

Candidates must hold an Associate's Degree in Nursing and a valid Registered Nurse license. Additionally, at least 3 years of clinical nursing experience and a valid driver's license are required.
