Job summary

The RN Case Manager is responsible for delivering high-acuity home health care, including wound care and infusion therapy, conducting patient assessments, managing care plans, coordinating services, administering medications, and providing patient education and supervision, primarily in a field-based, autonomy-driven environment.

Responsibilities

The RN Case Manager will own patient assessments, documentation, and the development of care plans while delivering high-acuity home health services. They are responsible for coordinating care across disciplines, advocating for patients, and providing supervision to LVNs and HHAs.

Qualifications

Candidates must hold a current Texas RN license and possess at least 3 years of clinical nursing experience. Proficiency in wound care, infusion therapy, and valid BLS certification are required for this field-based role.
