Responsibilities

The RN Case Manager plans and directs hospice care, performing comprehensive patient assessments and developing individualized care plans. They coordinate services with an interdisciplinary team to meet the physical and emotional needs of patients and their families in home settings.

Qualifications

Candidates must be graduates of an accredited nursing school with current state licensure and CPR certification. Required experience includes 1-2 years of recent acute care in an institutional setting and 1-2 years of recent home care experience.
