Job summary

Provides patient support including hygiene, nutrition, ambulation, elimination, and safety, following departmental guidelines. Performs documentation, admits residents, responds to resident needs, and maintains equipment and supplies.

Responsibilities

Provide direct resident care, including assistance with hygiene, nutrition, mobility, elimination, safety, admission procedures, and prescribed treatments. Document care and measurements accurately, respond to resident needs, maintain supplies, communicate with the Charge Nurse and care team, and complete shift handoffs.

Qualifications

An active Certified Nurse Assistant certification in the state of employment is required; CPR certification is required or must be obtained within six months. A high school diploma and long-term care experience are preferred, along with strong communication and organizational skills and the physical ability to assist and transfer residents and handle equipment.
