Responsibilities

The RN Case Manager provides end-of-life care and hospice services to patients in their homes or community settings. They are responsible for assessing patient needs, developing care plans, and collaborating with the interdisciplinary team to ensure high-quality, compassionate care.

Qualifications

Candidates must hold a current, unrestricted RN license in the state of practice and possess a valid driver's license. A minimum of two years of RN experience is required, with preference given to those with hospice or palliative nursing experience.
