Job summary

Licensed Practical Nurse (LPN) providing skilled nursing care in patient's homes, following physician orders, monitoring patient conditions, administering medication, and educating patients and families, with a focus on positive communication, documentation, and teamwork. Must have valid LPN license, at least one year of acute care experience, and a valid driver's license. Home care experience preferred.

Responsibilities

Provides skilled nursing care in patient homes by following physician orders and care plans developed by the RN. Monitors patient conditions, administers medications, and completes all clinical documentation according to Medicare and agency guidelines.
