Job summary

The Licensed Practical Nurse (LPN) at a rehabilitation hospital performs patient assessments, initiates interventions, provides education, documents care, and collaborates with a multidisciplinary team in a fast-paced inpatient setting.

Responsibilities

Assess patients, identify care needs, initiate appropriate nursing interventions, and provide age- and condition-appropriate care throughout rehabilitation. Educate patients and families, collaborate with the interdisciplinary team, maintain care plans and medical records, and participate in quality improvement activities.

Qualifications

Critical Thinking Decisive Judgement Documentation Interdisciplinary Care Planning Minimal Supervision Patient Assessment Patient Education Quality Improvement Stress Management

An active Wisconsin or compact-state LPN license and an associate degree are required; AHA Basic Life Support certification must be obtained within 30 days of hire. One year of prior experience is preferred, along with critical thinking, sound judgment, and the ability to work with minimal supervision in a stressful environment.
